Terms of Service

1. Acceptance of Terms
By choosing Phoenix House as your assisted living facility, you agree to be bound by these Terms of Service. These terms constitute a binding agreement between you (the "Resident" or "Responsible Party") and Phoenix House.
2. Services Provided
Phoenix House provides assisted living services including but not limited to:
- 24-hour staffing and supervision
- Assistance with activities of daily living
- Medication management
- Three nutritious meals daily and snacks
- Housekeeping and laundry services
- Social and recreational activities
- Transportation to medical appointments
- Emergency response systems
3. Admission Requirements
Admission to Phoenix House is subject to:
- Completion of all admission documentation
- Medical evaluation and approval
- Financial qualification
- Availability of appropriate accommodations
- Our ability to meet the resident's care needs
Phoenix House reserves the right to deny admission if we determine we cannot adequately meet a potential resident's needs.
4. Resident Rights and Responsibilities
Rights
Residents have the right to:
- Be treated with dignity and respect
- Privacy in personal care and medical treatment
- Make choices about their care and daily routine
- Voice grievances without fear of retaliation
- Have visitors at reasonable times
- Participate in community activities
Responsibilities
Residents are expected to:
- Treat staff and other residents with respect
- Follow community rules and guidelines
- Pay fees in a timely manner
- Provide accurate health information
- Notify staff of changes in health condition
- Respect the property of others
5. Fees and Payment
Monthly fees are due on the first day of each month and cover:
- Room and board
- Basic care services as outlined in the care plan
- Meals and snacks
- Housekeeping and laundry
- Scheduled activities
Additional fees may apply for:
- Enhanced care services
- Personal transportation
- Guest meals
- Personal care supplies
Late payments may incur additional fees. Continued non-payment may result in discharge from the facility.
6. Discharge Policy
Phoenix House may discharge a resident for:
- Non-payment of fees
- Care needs exceeding our capabilities
- Behavior that endangers self or others
- Repeated violation of community rules
- Medical needs requiring a higher level of care
Except in emergencies, we will provide 30 days written notice before discharge.
7. Liability and Insurance
Phoenix House maintains liability insurance and takes reasonable precautions to ensure resident safety. However, residents are encouraged to maintain personal property insurance. Phoenix House is not responsible for lost or stolen personal items unless due to our negligence.
8. Medical Care
Phoenix House is not a medical facility. We coordinate with healthcare providers but do not provide medical treatment. Residents are responsible for their own medical care decisions and expenses. In emergencies, we will call 911 and notify the designated emergency contact.
9. Visiting Hours
Family and friends are welcome to visit. While we encourage visits during reasonable hours (8 AM - 8 PM), we understand that special circumstances may require flexibility. All visitors must sign in at the front desk and respect the privacy and comfort of all residents.
10. Dispute Resolution
Any disputes arising from these terms shall first be addressed through our internal grievance process. If not resolved, disputes may be subject to mediation or arbitration as agreed upon by both parties.
11. Changes to Terms
Phoenix House reserves the right to modify these terms with 30 days written notice to residents and responsible parties. Continued residency after such notice constitutes acceptance of the modified terms.
